If Yes!
Then you are ready to USPS mail, fax or email the completed questionnaires
and forms on this Webpage (Webpage D) to Dr. Smith.
And if you have not already done so, remember that the two "Informed Consents" and "CPA" form on Webpage C must be signed, initialed and
then mailed to Dr. Smith by USPS or other Postal Carrier.
Once the above has been done, after you had your blood drawn and/or mailed your hair, urine, saliva or stool specimen sample(s) to our Labs, whichever applies to you,
you are then ready to return to "New Client Instruction" link in the Left Column
and read the information on Pop-Out Webpages E, F and G before your
Report of Findings (RF) Telephone Consultation appointment.

The RF Consultation is scheduled once Dr. Smith is ready to discuss her Clinical Nutrition Findings after receiving the Lab Reports from testing your genetics or biochemistry (blood, urine, hair, saliva or stool) AND the Questionnaires and Forms you have completed and submitted from the "New Client Instructions - Webpages C and D."
Please note:
If you are unable to read everything on Webpages E, F, G before the RF appointment,
that is okay, just complete them as soon as possible.
