Part 1 of the
"Specimen Collection" instructions to prepare for, collect and mail your specimen samples to our Labs and/or having blood drawn. Reading this information before you receive your Lab Kits and/or Lab Requisitions will save time, and you will be ready when they are received for
Part 2 of the Specimen Collection instructions, which is included:
- in the actual Lab Kits that you receive by mail from us, when having hair, saliva, urine or stool testing and/or
- in the Lab Requisition email, when having blood testing.
D - QUESTIONNAIRES Webpage "D" provides PDF documents of each
Health History and Symptom Questionnaire and Form to complete and sent to us by email, fax or U.S. mail, within 10 days after mailing your samples to the their Labs and/or having blood drawn. Properly completing and receiving these questionnaires and forms are required prior to the analysis of Lab Reports and the subsequent releasing of Lab Reports to clients.
