Clinical Nutrition Therapeutic and Health Maintenance Services
To the left and right are three to four Health Appraisal Comprehensive (HAC) Reports for two clients, one female 46 years, and one male 56 years. The HAC Report is a scientific evaluation of their symptoms. This report is based on research from Cornell University's Medical Index for Nutritional Symptoms. The HAC Report presents 28 Evaluations within the Ten Biological Systems in both percentages and categories of severity from mild, moderate to severe, in a Summary Bargraph format. For example in the Cardiovascular System, symptoms of Anemia, Hypertension, Heart and Circulation are evaluated. Please note that some of the images are a bit blurred, however, you will still be able to read the reports.

For businesses like ours, who do not physically swipe your credit or debit card into card payment processing machine to obtain a point of sale receipt for your signature, to acknowledge your approval for payment, the law requires the following two-step process:
Step 1:
We will send you an email and the Subject Line will read: "Card Authorization Email from Advanced Clinical Nutrition - Your Name."
Upon receipt of this email, read the contents and then simply click the REPLY and SEND buttons to send the email back to us. By sending this email back to us you are giving us your written permission to charge your card for the Analysis Fees for the Clinical Nutrition Biochemical Tests and Evaluations agreed upon at your FREE Inquiry Telephone Consultation.
Step 2:
Click on the red PDF button to the right and then print the "Card Payment Authorization" (CPA) form.
Complete all the information requested on this form. (Fill in the blanks).
Sign and date the CPA Form as requested.
Photo copy the front and back of your debit / credit card.
Then either Scan/Email or Fax your signed completed Card Payment Authorization form and Photocopy of Card to us.
Lastly, please mail the originally signed CAF document and photo copy of card through your regular postal service, keeping a copy of each for your files.
Please allow 5-7 business days to receive your Welcome Email, Lab Kit Package from USPS, and emailed Blood Chemistry Lab Requisition, when blood is also ordered for your Initial Assessment. Thank you and Welcome!
WHY 2 Steps?
Doing Step 1 allows us to get started right away processing your Initial Assessment so you can start therapy as quickly as possible. However, because the law requires that we obtain your physical signature, you will also need to do Step 2 immediately following Step 1. In other words, Step 1 gets the ball rolling as quickly as possible and Step 2 keeps the ball rolling. Our website does not have a shopping cart because it is what is termed an "information only" website. So our website has no need for a padlock. Regardless of the reason, they are unable to add the padlock to our secure website because it is an older web design from the 1990's.
